Comparison of Quantitative Image Quality of Brain CT between Head-first and Feet-first Position with an Emphasis on Image Sharpness

Abstract
Brain CT scans are usually run in the head first position(HFP). Critical emergency patients may be prescribed for several areas at the same time during a CT scan of the head. And as the moving range of the examination table is extended, high-risk medical devices that are applied to patients may be eliminated. In this case, the examination posture direction is sometimes in the feet first position(FFP), which has been avoided due to the possibility of beam hardening artifacts. Therefore, this study evaluated sharpness using image noise and plot profile for each of 20 patients who underwent the examination in the HFP and the FFP to compare the image quality in two posture directions. In retrospective study, the noise results of the basal ganglia and the cerebellum 2 ROI in the center of the petrous bone were presented in the order of the HFP and the FFP. And they were 3.64¡¾0.35 vs 3.89¡¾0.44, 4.79¡¾0.58 vs 4.96¡¾0.51 in sequence mode, and 3.51¡¾0.24 vs 3.89¡¾0.43, 4.79¡¾0.58 vs 4.99¡¾0.48 in helical mode. The length of half-width(FWHM) of the X-axis section increasing from the minimum value to the maximum value by quantifying based on the line density profile was 1.47¡¾0.39 mm vs 1.56¡¾0.38 mm in sequence mode and 2.02¡¾0.53 mm vs 2.16¡¾0.55 mm in helical mode. As a result, In the HFP and the FFP image, the difference in length between the noise and the half-width was not significant, so it is considered that the posture of the FFP is possible when necessary in emergency critical patients and clinical situation.
